C&H Supportive Living Services is designed to assist each consumer to fulfill and maintain a healthy and safe lifestyle.

Our mission is to provide supportive services that will allow opportunities to consumers with intellectual abilities to live successfully in their choice of homes, exercise the maximum possible control over their lives, develop self-esteem, develop positive relationship, and become significant members of their community.

Our goal will always be in keeping with each consumer’s Personal Center Plan.

It is our objective to help every consumers acquire practical skills for home and in the community, and to achieve appropriate social and emotional skills and self-control.

C&H Supportive Living Services will provide a healthy atmosphere of care, love and acceptance.

Objectives
The Objectives of C&H Supportive Living Services are based on the beliefs that all persons are unique and have diverse needs. We respect unconditionally respect each person’s uniqueness and the gifts that each one brings to our attention. We also believe that they have the rights to choose services and support that will enhance their success in integrating into the community.

C&H Supportive Living Services are tailored to meet those specific needs of a consumer that cannot be met because of the nature of severity of a physical or developmental challenge. Personal support service includes, but is not limited to, any services or assessment or service needs, that support a consumer.
